Jo Hikaka is a clinical pharmacist with 15 years experience. For the past 7 years she has worked in multidisciplinary teams in the Residential Aged Care setting, advising on medicine optimisation in frail older adults, working to improve healthcare for residents and whānau.
Deprescribing is a term which describes the active process of reviewing medicines prompting medicines to be stopped or reduced if they now pose more potential risk to an individual, than therapeutic benefit. Jo will discuss the concept of deprescribing in older adults, barriers to deprescribing and strategies that can be used to overcome these barriers so that medicine burden can be reduced.
